Pregnancy is different for every woman, not all the same rules apply.
My brother was born in 1965, my mom was a chainsmoker and kept it up throughout her pregnancy with him. Back in the 60's, lots of docs strangely enough encouraged women to smoke in order to keep their weight down (mom gained less than 20 lbs with my brother). My brother was OK, except for his allergies. Really bad allergies to dust, most animals, dyes, soaps. There is no doubt in my mind that was attributed to my mother smoking through her entire pregnancy with him, and my brother's doc believes this, even my mom believes this to be as well. I got lucky, my mom made an effort and quit smoking while pregnant with me - then again that was the 70's and things had changed since the 60's.
As kids we used to also swing on monkey bars over hard concrete, walk to and from school unattended by an adult, ride bikes without helmets, ride in cars without car seats, and we were given aspirin to help bring down fevers. Not saying it's right in this day and age, but it's amazing how we all survived, isn't it?
Smoking while pregnant won't cause the baby to be born with gills and flippers - immediate effects of smoking while pregnant are primarily low birthweight. Still not wort the risk of other complications that can pop up and follow a child into adulthood.
Then again, with kids, it's always fuckin' something! ;)
